CAPABLE (Community Aging in Place, Advancing Better Living for Elders) is a free, evidence-based program that helps older adults live safely and independently in their own homes. Through personalized support from an occupational therapist, registered nurse, and handy worker, participants improve mobility, safety, and daily functioning while reducing risks that can lead to hospitalization or institutional care

CAPABLE (at JFS sponsored by Beth Sholom) is a free program to help older adults function safely in their homes. Teams comprised of an Occupational Therapist, a Registered Nurse, and a Handy Worker help individuals identify their own goals which can allow them to remain in their homes for longer.

The CAPABLE team evaluates the home environment and works to improve medication management, problem solving ability, strength, balance, mobility, nutrition, and home safety while decreasing isolation, depression and fall risk. The services are offered in the home during 10 visits over 4 months.

Eligibility

  • 60 years of age
  • Challenges with activities of daily living (i.e., bathing, dressing, cooking, walking, etc.)
  • Ability to actively participate
  • Meets income requirements
  • Richmond area resident
