Options for Independence

Recognizing you or a loved one needs assistance managing life doesn’t have to mean a loss of independence. In fact, it can be empowering to have options to choose from that help support your needs and wellbeing.

JFS offers each client an individualized Care Management plan based on a comprehensive assessment. Our Care Management staff is made up of social workers (LCSW, MSW and BSW), Geriatric Care Managers, nurses and professional nursing assistants, who work with you and discuss services you would like to receive to enhance your quality of life. Once a plan is set in motion, we review it regularly to ensure your needs and expectations are being met.

We are committed to providing services with the utmost care and respect for a person’s wishes. While we’re able to support an individual wanting to remain in the home for as long as possible, we’re also experienced in having the difficult conversations when home is no longer a safe option and can identify options to help with the transition.

In any environment, our dedicated Care Management staff works with you and your family to advocate for a person’s best quality of life.

Our Care Managers can help with:

• Assessing your situation and creating a personalized care plan

• Coordinating services at JFS and your other service providers, including physicians, counselors, attorneys, accountants, insurers and other professionals

• Acting as a liaison for families caregiving locally or from a distance

• Participating in medical appointments and acting as an advocate and medical historian

• Assisting with transitions to a retirement community, assisted living facility, or nursing home

• Providing bill paying and budgeting (see below)

• Maintaining your home, including pet care, coordinating routine upkeep and repairs, yard care, pest control, weather related prep and response and home safety monitoring and modifications

• Taking the role of guardian, conservator, power of attorney or advance medical directive agent

About the JFS Bill Paying Program

One of the many services offered at Jewish Family Services is our Bill Paying Program. This service allows JFS to assist with taking the day to day financial responsibilities off your hands, reducing your stress, while still keeping you informed. Our staff ensures you never have to worry if your utility payment is sent on time or if you remembered to pay that doctor’s bill. We maintain personalized financial records that can be helpful with budgeting and provide necessary information at tax time, and we can help recognize changes in reoccurring service invoices and explore reasons for the change. Our staff can also be a resource for information and referrals for additional services and help to mitigate the opportunity for scams and frauds which have become all too common these days. With the JFS Bill Paying Program, make your financial wellbeing our number one priority.

 

Guardianship at JFS

When an individual is no longer able to make their own decisions and needs someone else to ensure their best quality of life, JFS can help through our Public and Private Guardianship programs.

Guardianship is a court appointment made when an individual does not have a Power of Attorney or other decision maker or that decision maker is no longer able to help. JFS can provide you with information to consider and the steps to proceed if guardianship may be the right choice.

 

Private Guardianship

The Private Guardianship program offers the same comprehensive coordination of care and services found in JFS's Care Management program. With our person centered approach, we address the individual's health needs as well as their emotional needs to ensure the comfort and care essential to a person’s high quality of life.

Public Guardianship

Established in 2006 and funded by Virginia Division for the Aging, the JFS Richmond Public Guardianship Program serves as a program of last resort for vulnerable adults residing in the Cities of Petersburg and Hopewell; and the Counties of Goochland, Powhatan, Hanover, Prince George, and Sussex.  Basic eligibility requires that the person be an indigent, incapacitated adult who cannot be served by any less restrictive means, and has no other interested party willing or able to serve as a surrogate decision maker.

Aging Life Care Association

JFS Richmond is a member of the Aging Life Care Association. Aging Life Care™, also known as geriatric care management, is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals provides the answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off of work for family caregivers through:

  • Assessment and monitoring

  • Planning and problem-solving

  • Education and advocacy

  • Family caregiver coaching
