Care Manager

JFS is looking for a motivated Care Manager to provide high-quality services to adults and families. Care Managers are social work professionals working with a variety of people, generally in life changing transitions or events. They assess client needs, and then develop, implement and monitor service delivery and assistance plans, including care coordination with services from other agencies, maintaining records and files, and preparing reports. The Care Manager is responsible for providing information, support and services to clients and their families in a timely, professionally ethical and compassionate manner. They provide support to the agency and programs in a manner that is culturally sensitive to the service population.
Duties and Responsibilities:
The Care Manager’s responsibilities include but are not limited to the following:
Client Services:
- Address intake calls with potential clients and provides accurate and detailed information
- Complete comprehensive assessments, services plans and quarterly reviews.
- Provide comprehensive care management services to clients
- Collaboration and engagement with community resources available to support and meet the needs of older adults and adults with disabilities, as well as the needs of families and caregivers.
- Engagement with the current standards and criteria set by JFS for serving in the role as Guardian, Conservator, and Power of Attorney, as well as a comprehensive understanding of the legal criteria for eligibility.
- Knowledge of entitlements, eligibility guidelines and the application process.
- Maintain a balanced caseload and meet billable expectations.
- Work in partnership with the Care Management team and interns to meet clients’ needs.
- Participate in on-call rotation.
Compliance:
- Professional, timely and accurate completion of case file entries and documentation..
- Ongoing review and updates of client’s care plan
Program Needs:
- Networking and collaboration with community partners to facilitate Care Management client services
- Promoting JFS Care Management services to increase awareness and obtain new client referrals.
- Providing support and consultation to Interns.
- Attendance and active participation at all agency and departmental meetings.
- Other assignments as determined by Chief Program Officer and Care Services Director.
Qualifications:
- Bachelor’s in Social Work or other Human Services degree required, Master's in Social Work Preferred
- Familiarity with providing comprehensive case management with older adults and adults with disabilities
- Familiarity with age-related health issues, dementia, grief and loss and multidimensional caregiving
- Personal skills needed to be successful as a Care Manager are creativity, flexibility, organizational skills, time management and the ability to work both independently and as a team player.
- The ideal candidate will have experience working with seniors and adults with disabilities. The ability to effectively communicate and serve as a liaison between the client, families served, and various community agencies is of the utmost importance.

Pay: $50,000.00 - $57,000.00 per year
