If needed, we can even take the role of private guardian and/or conservator.

If you need someone to help you handle things on your own and support your independence, or if your aging parents require extra attention, our Home Care team can help with assistance in daily life on a regular or as-needed basis.

If your situation is unique and you're not sure what you need, our Care Management staff can conduct an individual assessment and create a personalized care plan to enhance your goal of independence, exclusively for you or your family.

And if you're simply looking for someone to check in on you or a loved one, our Senior Engagement program can arrange Friendly Visitors, as well as calls from Telephone Reassurance volunteers at no charge to you.

Assistance with Daily Life

Life can be hard. When you're older or have special needs, it can be especially difficult. If you're a caregiver, the responsibilities can be overwhelming. In times like this, we all need a little help. Fortunately, you have somewhere to turn.

JFS provides high-quality home care services delivered right to you or your loved one's doorstep. Our Home Care team is made up of professional nursing assistants and nurses (both RNs and LPNs), all screened and trained to ensure top quality care. As a testament to our dedicated service, we handle unique assignments, regardless of the complexities and challenges.

We work hard to build thoughtful, considerate relationships with both you and your loved one based on mutual trust, empathy and respect. Whether assistance is needed for as little as four hours per day, or as much as 24/7, we can give you the help you need.

As a licensed home care organization, JFS meets state and federal regulations for quality home care and adheres to the highest standards of excellence.

Our nursing assistants can help with:
• Bathing, dressing, grooming
• Meals and nutrition support
• Mobility and transfers
• Toileting
• Medication reminders
• Companionship and respite care

Additional services are also available from our Home Care team such as light housekeeping, laundry, scheduling appointments, running errands and even social outings.

Care Management

Options for Independence

Recognizing you or a loved one needs assistance managing life doesn’t have to mean a loss of independence. In fact, it can be empowering to have options to choose from that help support your needs and wellbeing.

JFS offers each client an individualized Care Management plan based on a comprehensive assessment. Our Care Management staff is made up of social workers (LCSW, MSW and BSW), Certified Geriatric Care Managers, nurses and professional nursing assistants, who work with you and discuss services you would like to receive to enhance your quality of life. Once a plan is set in motion, we review it regularly to ensure your needs and expectations are being met.

We are committed to providing services with the utmost care and respect for a person’s wishes. While we’re able to support an individual wanting to remain in the home for as long as possible, we’re also experienced in having the difficult conversations when home is no longer a safe option and can identify options to help with the transition.

In any environment, our dedicated Care Management staff works with you and your family to advocate for a person’s best quality of life.

Our Care Managers can help with:

• Assessing your situation and creating a personalized care plan

• Coordinating services at JFS and your other service providers, including physicians, counselors, attorneys, accountants, insurers and other professionals

• Acting as a liaison for families caregiving locally or from a distance

• Participating in medical appointments and acting as an advocate

• Assisting with transitions to a retirement community or assisted living facility

• Providing bill paying and budgeting (see below)

• Maintaining your home, including pet care, coordinating routine upkeep and repairs, yard care, pest control, weather related prep and response and home safety monitoring and modifications

• Taking the role of guardian, conservator, power of attorney or advance medical directive agent

About the JFS Bill Paying Program

One of the many services offered at Jewish Family Services is our Bill Paying Program. This service allows JFS to assist with taking the day to day financial responsibilities off your hands, reducing your stress, while still keeping you informed. Our staff ensures you never have to worry if your utility payment is sent on time or if you remembered to pay that doctor’s bill. We maintain personalized financial records that can be helpful with budgeting and provide necessary information at tax time, and we can help recognize changes in reoccurring service invoices and explore reasons for the change. Our staff can also be a resource for information and referrals for additional services and help to mitigate the opportunity for scams and frauds which have become all too common these days. With the JFS Bill Paying Program, make your financial wellbeing our number one priority.

Senior Engagement

For many as they grow older, it’s easy to feel isolated. Lack of mobility, illness and other challenges of aging can often mean less opportunity for interaction and socialization. These years should not be a time of solitude and detachment.

 

JFS understands that even the smallest gesture of kindness and empathy can go a long way in bettering the lives of those in need. That's why our Senior Engagement program was created. Our Friendly Visitors and Telephone Reassurance volunteers ensure seniors in our community are not forgotten.

 

Friendly Visitors make weekly social visits with older or homebound adults. These important volunteers offer friendship and companionship to individuals looking for someone to spend time with. They share stories, listen and reminisce, play games, take walks and simply enjoy each other's company.

Telephone Reassurance volunteers call every weekday morning to check in, chat and make sure everything is okay. These dedicated volunteers help start the day of an isolated individual on the right foot. They also offer peace of mind for those living alone.
