Here’s what Elderly Care Management can do for you:

• Assess your loved one’s situation and create a personalized care plan
• Coordinate your loved one’s care with physicians, counselors, attorneys, accountants, insurers, and other professionals
• Act as liaison for families caregiving at a distance or locally
• Help your parents pay their bills and manage their finances
• Drive and accompany your loved one to doctors’ appointments
• Visit your loved one to monitor health and safety
• Arrange for home care, if needed
• Assist with moving your parent to a retirement or assisted-living facility, if needed
• Act as private guardian if needed
• Communicate with you regularly

Caregiver Support Group: JFS holds a free Caregiver Support Group on the third Thursday of each month from 2:00 p.m. to 3:30 p.m. All caregivers are welcome to join us.
